Philips and a consortium of European health care regions, companies, universities and hospitals have announced a three-year, large-scale coordination and telehealth program to help thousands of people living with chronic conditions in the European Union (EU).

The program, ACT@Scale, will collect and analyze health outcome and economic impact data for large populations of chronic patients and elderly people in order to develop and make facilities’ coordination and telehealth programs more effective.

The program will gather information from more than 75,000 patients in the UK, the Netherlands, Spain and Denmark, and these countries will share a standardized data set that includes program outcomes such as the number of patients included, hospitalizations, duration of stay and mortality rates. They will also assess economic factors such as cost per patient and the patients' impact on hospitals' income models.

The gathered information will support the development of new and sustainable business models, according to the announcement, and patient satisfaction scores will be measured to determine how communication through technology empowers them and affects overall health.

“Philips is committed to helping care providers and governments implement innovative, connected care delivery models that help improve patient self-management and care collaboration,” said Jeroen Tas, CEO of connected care and health informatics at Philips, in a statement.

“The ACT@Scale program will provide the evidence needed to successfully deliver a seamless patient experience with better outcomes at lower costs,” he said.

The following countries are participating in the program:

  • Catalonia, Spain, which developed programs to support nursing homes, reduce hospital readmissions, manage complex cases and promote physical activity
  • Southern Denmark, which will have a telehealth program to help in psychiatric treatment
  • Northern Ireland, which has remote telemonitoring programs to support patients with chronic obstructive pulmonary disease (COPD) and diabetes, and manages maternal obesity
  • Northern Netherlands, with programs to provide specialist support for COPD, asthma and heart failure patients
  • Basque Country, Spain, which will have programs to support older individuals with health and social care needs, and telehealth services for chronic heart failure patients

ACT@Scale is based on the ACT program, a two-and-a-half-year study that investigated the results of integrated health programs in Europe, and conducted thousands of interviews with patients and care providers.

“Telehealth and coordinated care services may offer the elderly and otherwise frail individuals the ability to maintain their independence for longer and enjoy a significantly better quality of life,” said Erik Buskens, professor of medical technology assessment at the University Medical Center Groningen, in a statement.
